Hello Sir/Ma'am,
The patient has showed little signs of improvement in the past six months. The medications had multiple side effects on the patient, including nausea, migraines, sleepiness, lack of appetite and bloated weight gain. Mr Jefferson prescribed different medications which seem to be working well.
The patient has become quite stubborn and refuses to speak to anyone except for Mr Jefferson. Will not say why or explain feelings. The patient has not learned yet to compete trust Mr Jefferson, and does not participate in therapy, but only sits and plays with plush toy bear.
Toy bear was given to patient by parents, and becomes extremely agitated when separated from it. Nurses are on strict orders to not touch the bear. Mr Jefferson fears that separation from such bear will send patient into a downward spiral, yet again.
The patient has yet to improve on physical and mental health. We are extending her stay yet again. This time for another six months.
We are beginning to think that the patient is going to be a long term case.
